Job Description I am a nurse practitioner and business owner looking for an experienced CRM automation specialist to help build automated patient follow-up workflows for my medical practice. The goal is to automate routine patient check-ins while ensuring that clinically important responses are escalated to me. This project will involve building automated messaging workflows, response routing, and a website chatbot for patient inquiries. Experience with healthcare workflows, HIPAA-compliant tools, and CRM automation platforms is strongly preferred. Project Scope 1. Automated Patient Follow-Up Workflow Create a system that: • Sends automated follow-up check-ins to patients at intervals I set (for example: every 2 weeks initially, then every 4 weeks). • Messages should contain structured follow-up questions about treatment progress. • Patients respond through text or email Example workflow: Patient receives automated follow-up message Patient answers questions Responses are automatically summarized and created as a note in the EMR or patient record If responses meet certain criteria (for example: side effects, worsening symptoms, concerns), the system flags and notifies me for manual follow-up. 2. Conditional Escalation The system should allow: • Trigger alerts when patients answer certain questions in a concerning way • Notify me via email, CRM alert, or SMS • Tag those patients as “Needs Provider Follow-Up” 3. EMR / Documentation Integration When patients respond: • Their answers should be automatically logged as a patient follow-up note • Ideally structured in a format that can be copied directly into the EMR If direct EMR integration is not possible, a structured summary output would be acceptable. 4. Website Chatbot I also want a chatbot on my website that: • Answers basic patient questions • Provides information about services and pricing • Captures leads and contact information • Sends inquiries to my email or CRM • Optionally books consultations
