I am a chiropractic physician who performs detailed initial evaluations for patients. My workflow currently involves completing a structured handwritten intake form and then manually transferring that information into a separate Word (.docx) narrative template. I am looking for a developer to build a reliable, repeatable automation tool that eliminates the manual transcription step. The task: • I upload a scanned image or PDF of a completed handwritten intake form. • The system extracts the relevant data fields (OCR + structured extraction). • The system populates my existing Word (.docx) template exactly as written. • The final output is a properly formatted Word document ready for review and printing. This must be a mechanical transfer process, not an AI summarizer. The template wording, structure, headings, and formatting must remain unchanged. Accuracy, formatting consistency, and repeatability are extremely important because this is used in a medico-legal setting. What I’m looking for in a working relationship: • Clear communication • Structured milestone-based progress • Someone who understands document automation and can propose a clean technical solution • Clean, maintainable code • A short usage guide once complete This is a one-time build.
