Remote | Contract | Full Lifecycle Case Coordination
We are a healthcare coordination infrastructure platform. We are not a clinical provider and do not deliver medical care. We operate as a structured coordination and routing layer connecting users to licensed providers, public benefit programs, and durable medical equipment (DME) vendors.
We operate using risk-based triage, defined service-level agreements (SLAs), and structured case lifecycle accountability.
We are currently stabilizing and strengthening our intake and submission verification workflows as we scale.
Role Overview
We are seeking experienced Non-Clinical Health Care Workers to manage full lifecycle case coordination.
This role includes:
Intake validation
User outreach
Benefits application assistance
Submission verification
Case tracking and follow-up
Escalation of high-risk cases
This is not a clinical position and does not involve medical decision-making.
Core Responsibilities
Review and validate intake records (bot and referral sources)
Contact users to verify identity and clarify needs
Assist with Medicaid, SNAP, LIHEAP, housing, and related benefit applications
Ensure applications are completed accurately and fully
Confirm successful submission and document verification
Track cases against SLA timelines
Escalate urgent cases (utilities shutoff, food insecurity, DME needs, elderly risk)
Maintain detailed documentation in CRM system
Follow structured workflow and escalation protocols
Required Qualifications
3+ years experience in healthcare coordination, benefits navigation, case management, or social services
Strong familiarity with Medicaid and public assistance programs
Experience working with high-risk or elderly populations
Strong documentation discipline
Experience working in CRM systems (Salesforce preferred)
Excellent English communication skills (written and verbal)
Preferred Qualifications
Spanish-English bilingual
Experience with hospital discharge coordination
Experience handling DME or high-dependency equipment cases
Experience working in SLA-driven environments
Performance Standards
This is a metrics-driven role.
Performance will be evaluated on:
Time to first human contact
SLA compliance rate
Verified submission accuracy
Documentation completeness
Escalation appropriateness
Case resolution timeliness
Accuracy and integrity are prioritized over speed.
Compensation
This is a contract role.
Candidates must submit their proposed hourly rate.
Rates will be evaluated based on:

Language proficiency
Workflow discipline

English fluency is required.
Spanish-English bilingual candidates strongly preferred.
