Project Summary
We have a Pressure Proof Assessment built in Typeform and a QR code that drives traffic to the assessment.
We need a vendor to integrate Typeform → HubSpot so that when someone completes the assessment, they are:
Created/updated as a contact in HubSpot
Automatically sent an email that delivers their results
Set up in a way that does not block our future plan to embed/launch the assessment as a lead magnet on MelissaDawnSimkins.com and route people into a HubSpot nurture sequence.

• QR code already in use
• We will provide:
Access to Typeform and HubSpot
The PDF results output/template
The final email message copy (if results are delivered via email)
Core Requirements (Must-Haves)
A) Integration + Data Mapping
• Connect Typeform submissions to HubSpot (vendor to recommend native integration vs Zapier/Make).
• Map Typeform fields into HubSpot contact properties, including at minimum:
First name (if collected)
Last name (if collected)
Email (required)
Any scoring fields required to generate and segment results
Assessment completion timestamp
Source tracking field (ex: “Pressure Proof Assessment – QR”)
• Ensure existing contacts are updated (not duplicated).
B) Results Email Automation
Build a HubSpot workflow that triggers immediately after assessment completion.
Workflow must work for both new and existing contacts.
Results email must be sent automatically once the assessment is completed.
Important:
We will provide the final email message copy and structure if email is the delivery mechanism for results. The vendor is responsible for:
Building the email inside HubSpot
Inserting dynamic personalization tokens as needed
Connecting the workflow trigger correctly
Testing full delivery and formatting
Vendor should recommend best practice for results delivery:
Structured results inside the email body
Link to results page
PDF attachment (if technically advisable)
C) Website Lead Magnet Readiness (Future-Proofing)
This setup must allow for two future entry points without rebuilding logic:
QR code → Typeform assessment
Website lead magnet (MelissaDawnSimkins.com) → assessment → HubSpot sequence
Requirements:
Workflow must support multiple source values (QR and Website)
Architecture must be flexible enough to embed the Typeform later or replace with a HubSpot form without breaking automation
Clear tagging/segmentation for future nurture sequencing
Deliverables
1. Working Typeform → HubSpot integration with proper field mapping
2. HubSpot workflow(s) that automatically send results email
3. Results email template built in HubSpot (using our provided copy)
4. End-to-end testing completed

- Walkthrough of integration, workflow, and email setup
- Final handoff notes delivered
Acceptance criteria:
- Client can repeat process independently after demo
