We need a Jotform specialist to create a fully automated system where drivers can submit loads, update ETAs, report incidents, upload documents, and request payments. All submissions must route to the employer automatically. Scope includes: • Build Jotform App portal dashboard • Create multiple forms (load request, ETA update, incident report, load advance, driver onboarding, compliance upload) • Configure email notifications and routing workflows • Set conditional logic and required fields • Connect forms to Jotform Tables (database) • Create organized backend tracking • Test submission flow and automation • Mobile-friendly setup
